Asthma Program

The goal of the Asthma Program is to reduce the burden of asthma across the state by focusing on two primary goals: (1) increasing education and awareness of people with asthma and their families (2) increasing capacity to provide guidelines-based comprehensive asthma care across the public health and healthcare sectors. The program focuses on expanding the use of strategic partnerships and communication, quality improvement and evaluation methods, school and home-based services, insurance coverage and reimbursement strategies, and providing culturally competent care.

Behavioral Risk Factor Surveillance System

The Ohio Behavioral Risk Factor Surveillance System (BRFSS) is a telephone survey used to collect data on health practices, health conditions and risk behaviors among Ohio adults ages 18 and older. BRFSS data are used to identify emerging health problems; track trends in health risk behaviors over time; and develop, monitor and evaluate public health programs and policies.

Breast and Cervical Cancer Project

The Ohio Breast and Cervical Cancer Project (BCCP) offers no-cost breast and cervical cancer screenings and diagnostic testing to qualified participants. BCCP can also assist all women with cancer screenings through its Patient Navigation Program (e.g., find providers and community resources, schedule appointments and answer questions about insurance).

Comprehensive Cancer Control Program

The Ohio Comprehensive Cancer Control Program uses an integrated and coordinated approach to reduce the incidence and mortality of cancer by preventing it, finding it early, treating it, and caring for those who have and survive cancer.

Coverdell Stroke Program

The Ohio Coverdell Stroke Program is a data-driven quality improvement program for stroke treatment that provides quality improvement resources to hospital stroke teams and other healthcare providers to reduce the number of Ohioans who die from or are disabled by stroke.

Creating Healthy Communities Program

The Creating Healthy Communities Program implements policy, system and environmental change strategies—statewide and in high burden communities—through cross-sector collaboration to improve access to and affordability of healthy food, increase opportunities for physical activity and assure tobacco-free living.

Diabetes and Heart Disease Prevention and Management Program

The Diabetes and Heart Disease Prevention and Management Program implements and evaluates evidence-based strategies to prevent and manage cardiovascular disease and diabetes in high-burden populations. High burden populations are those affected disproportionately by high blood pressure, high blood cholesterol, diabetes or prediabetes due to socioeconomic or other factors including inadequate access to care, poor quality of care or low income.

Ohio Cancer Incidence Surveillance System

The Ohio Cancer Incidence Surveillance System (OCISS) is Ohio’s central cancer registry.  OCISS collects information on all newly-diagnosed cancers in the state.  OCISS data are used to determine the burden of cancer in Ohio's communities and to target cancer prevention and early detection programs.

State Health Improvement Plan

The Ohio 2017-2019 State Health Improvement Plan (SHIP) was developed to improve the health of Ohioans through implementing a strategic set of evidence-based strategies at the scale needed to measurably improve population health outcomes and achieve health equity. Chronic disease is one of three priorities in the SHIP. Outcome objectives for chronic disease are to reduce heart disease, diabetes and child asthma through implementing activities that address healthy eating and active living; prediabetes screening, testing and referral to lifestyle change programs such as the National Diabetes Prevention Program; hypertension screening and management; medication therapy management and team-based care (e.g., use of community health workers) to control hypertension; asthma triggers in schools; home visits to improve self-management education and reduce asthma triggers, and access to tobacco cessation services. The SHIP takes a comprehensive approach to decreasing health disparities and achieving health equity by addressing the social determinants of health (education, housing, employment, etc.), prioritizing strategies most likely to reduce health disparities and implementing interventions that target priority populations.

Tobacco Use Prevention and Cessation Program

The goal of the Tobacco Use Prevention and Cessation Program is to prevent the initiation of the use of tobacco, to increase the number of people who quit using tobacco and to protect Ohioans from exposure to secondhand smoke. The program uses evidence-based strategies and interventions to achieve these goals and focuses effort on high-need populations disproportionately affected by tobacco.
